Living a Healthy Life with Chronic Conditions

Living a Healthy Life with Chronic Conditions is a six week workshop for anyone with a chronic condition. This could include diabetes, heart disease, arthritis, lung disease, and other chronic health issues. Anyone can register themselves for a workshop; no referral from a health care professional is needed. Workshops are highly interactive and focus on the topics below.

  • Getting a Good Night’s Sleep
  • Introduction to Action Plans

  • Feedback and Problem-Solving
  • Dealing with Difficult Emotions
  • Introduction to Physical Activity and Exercise
  • Preventing Falls and Improving Balance
  • Making an Action Plan

 

Session 3

  • Pain and Fatigue Management
  • Endurance Exercise
  • Relaxation: Body Scan
  • Making an Action Plan
